Due Dates & Forms

 

The Pottstown Area Health & Wellness Foundation Board of Directors has established semi-annual grant cycles in the Spring and Fall. Below is a list of timelines for upcoming grant cycles.

Due to our efforts in going “paperless”, please note changes below related to submission of Letters of Intent, Grant Applications, and Reports.

Application Due Dates for 2011/2012

All Letters of Intent and Final Grant Applications should be e-mailed to GrantsApplication@pottstownfoundation.org

Fall 2011 Grant Round – Application Due Dates:
Thursday, August 25th (by 5:00 p.m.) – Due Date – Letter of Intent to be submitted via e-mail
Week of October 3rd – Move Ahead & Turn Down letters e-mailed
Wednesday, October 19th - by 5:00 p.m – Due Date – Final Grant Applications to be submitted via e-mail
Week of November 28th – Final Approval and Turn Down letters e-mailed

Spring 2012 Grant Round – Application Due Dates:
Thursday, February 23rd (by 5:00 p.m.) – Due Date – letter of Intent to be submitted via e-mail
Week of April 2nd - Move Ahead & Turn Down letters e-mailed
Wednesday, April 18th (by 5:00 p.m.) – Due Date – Final Grant Applications to be submitted via e-mail
Week of May 28th - Final Approval and Turn Down letters e-mailed


Interim and Final Report Due Dates for 2011/2012

All reports are to be submitted electronically to GrantsApplication@pottstownfoundation.org

  • Final Progress Reports due for Fall 2010 grant round recipients - to be submitted via e-mail

Friday, December 16, 2011

  • Interim Progress Reports due for Spring 2011 grant round recipients - to be submitted via e-mail

Thursday, June 15, 2012

  • Interim Progress Reports due for Fall 2011 grant round recipients - to be submitted via e-mail
  • Final Progress Reports due for Spring 2011 grant round recipients - to be submitted via e-mail

Standard submittal dates for Interim & Final reports will always be the 15th of the month. If the 15th falls on a Saturday, the reports will be due Friday the 14th. If the 15th falls on a Sunday, the reports will be due Monday the 16th.
